Whole Health in Your Practice: Exploring New Paths to Patient-Centered Care at UW Health

May 23-24, 2017

Location: 1102 South Park St., Large Conference Room

Welcome to the Whole Health at UW Health Website!  This web page is designed to accompany the 2-day conference on the Whole Health model of care held May 23-34, at 1102 South Park.  The course was made possible through collaborative efforts of UW Health Employee Wellness, UW Integrative Health, and Wellness at The American Center.

This clinical education program is designed to empower participants to focus beyond disease management towards the creation of health and well-being — for patients and for clinicians and health care teams.  Whole Health emphasizes shifting the healthcare system from “What’s the matter?” to “What really matters?”  It is based on a similar program developed by the Veteran’s Health Administration, Pacific Institute for Research and Evaluation, and the UW Department of Family Medicine and Community Health Integrative Health Program.  It is part of the VA’s national commitment to establishing a Whole Health model of care throughout the system. The existing program has been taught by UW clinicians 56 times in the past 3 ½ years at VA medical centers nationwide.

The Whole Health Conference provides an opportunity to explore the Whole Health model and consider how the approach might advance the UW Health system.  Already, UW Health Employee Wellness is building on the Whole Health model to further support the health and well-being of employees. UW Integrative Health colleagues have incorporated Whole Health into integrative health consults at the UW Research Park Clinic and primary care visits at the UW Arboretum Clinic.  The Whole Health approach also informs wellness consults and health coaching at The American Center.

Learning objectives for the conference:

  • Reflect on how we, as members of the UW Health community, can most effectively offer care that is personalized, proactive, and patient-driven
  • Learn about co-creating a Whole Health experience for patients.  The Whole Health approach can be used in both inpatient and outpatient settings
  • Receive evidence-based tools and information to support personal health and well-being, as well as that of patients and colleagues. Health, well-being and resilience are integral to the Whole Health model
  • Experience key elements of Whole Health firsthand, including mindful awareness, ways to enhance relationships with patients, and what it is like to provide care in alignment with what really matters to the patient
  • Explore ways to enhance patient engagement through co-creating personal health plans around patient aspirations and core values
  • Effectively share health plans with other team members, recognizing that “it takes a village” to offer Whole Health care
  • Share experiences and wisdom as we engage in collaborative conversations about the future of healthcare and how to take action to make that future a reality
  • Reflect on how best to enhance well-being for UW Health employees, exploring ways this model can be incorporated into Employee Wellness
  • Brainstorm about around what we do well with Whole Health in UW Health, as well as areas where we can continue to grow
