This site is an early step in technical support for the development, and clinical application of a Community Health Practice Profile (CHPP) that will align community and population data with the academic mission of the UW Department of Family Medicine and Community Health.
Our vision is of improved clinical care which energizes faculty and residents, the development of an interactive tool to facilitate the application and education of population health in family medicine and excites students and residents.
This site is a work in progress and will evolve over time as new methodologies are envisioned and tested. Maps and additional data will be added to create more complete CHPPs for the different clinics.

More content will come over time as the CHPP educational group, and general grant comittee develop materials, maps, reports and data to support the mission of the site.

5-26-15: UW Department of Family Medicine requested a department name change from UW Department of Family Medicine to UW Family Medicine and Community Health. This change was approved and will be effective on July 1, 2015.
